Staff-flagged output — hallucinations, factual errors, stale answers, sources filed wrong. Mine these to tune prompts and SOULs. Each report snapshots what the model was given and what it produced, plus the model and token/cost context for the run. Wiki reports keep their snapshot here only — the alert email carries a link, not the text.
